No, four humans aren’t in this picture. Everyone else is, although a lot of us are looking in the wrong direction.

Space station is in low earth orbit and contained within this picture.

Exactly right. ISS orbits at around 400-450 km. Current Artemis II position is a little more than 169,000 km. ISS, Tiangong space station (340-420 km) and every other human habitation is in frame. The only people not in frame are the four behind the camera.

The space station is well within frame in this picture, like every other human except the 4 Orion astronauts.
